These are some great questions, and a good idea.

In our company, we've mapped 611 to go to our customer service.  If 
you're providing SIP connectivity, wouldn't that be who they want to 
reach?  We map 411 to Google's free service.  911 and 511 are mapped 
using a variable attached to the SIP account.  This is a totally manual 
process though, and an area code to service translation table would be 
awesome.

Not really, or at least not with Asterisk.  Assuming you have a variable 
of some sort for the SIP account's local area code 
(setvar=defaultareacode=602), then you'd just build a macro that does a 
lookup for the value of 511 dialed in 602.
